ABSTRACT

<p><b>OBJECTIVE</b>To study the value of MRI in the diagnosis of cerebral sparganosis in children.</p><p><b>METHODS</b>The MRI appearances of 17 children with cerebral sparganosis proven by pathology or serological test were retrospectively studied. The diagnostic accuracy rate of cerebral sparganosis was compared before and after knowing the imaging features of this disease.</p><p><b>RESULTS</b>Enhanced MRI was performed in 12 cases, and all of them demonstrated abnormal enhancement. Peripheral ring-type, tortuous beaded shape or serpiginous tubular shape enhancement were found in 8 cases. After analysis of follow-up MRI for 7 cases, a change in location and shape of lesions was found in 2 cases. Typical MRI patterns of cerebral sparganosis included abnormal enhancement such as peripheral ring-type, tortuous beaded shape or serpiginous tubular shape enhancement, and changes in location and shape of lesions in the follow-up MRI. The diagnostic accuracy rate for the 17 cases of cerebral sparganosis at the first visit, after first imaging examination and after several follow-up MR examinations were 0%, 11.8% and 28.6%, respectively. The diagnostic accuracy rate of this disease increased to 64.7%, 70.6% and 85.7% respectively after the radiologists were trained in the imaging characteristics of the disease.</p><p><b>CONCLUSIONS</b>MR findings in children with cerebral sparganosis are specific. Mastering correctly the typical imaging features of the disease can be very helpful in reducing the rate of clinical misdiagnosis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the value of proton magnetic resonance spectroscopy ((1)H-MRS) with two-dimensional (2D) chemical-shift imaging (CSI) in evaluating brain gliomas.</p><p><b>METHODS</b>Thirty-six patients with gliomas received examinations with 2D-CSI. The VOI of MRS included the tumor, peritumoral edematous and nonedematous areas, and the contralateral normal tissue. The changes of the metabolites in different areas were determined using 2D-CSI (1)H-MRS with SE sequence and the metabolite ratios were calculated.</p><p><b>RESULTS</b>Significant differences were found in the ratios of NAA/Cr, Cho/Cr and NAA/Cho between low-grade gliomas and contralateral normal brain tissue, and between high-grade gliomas and the contralateral normal tissue (P<0.01). The low-grade gliomas and high-grade gliomas differed significantly in the ratios of NAA/Cr, Cho/Cr and NAA/Cho (P<0.05). These ratios also showed significant differences between peritumoral edematous area and the glioma tissue, between the peritumoral edematous area and contralateral normal brain tissue (P<0.05), and between the peritumoral nonedematous area and the glioma tissue (P<0.05). Between the peritumoral nonedematous area and contralateral normal brain tissue, NAA/Cr and NAA/Cho ratios were significantly different (P<0.05) but the Cho/Cr ratio was similar (P>0.05).</p><p><b>CONCLUSIONS</b>MRS with 2D-CSI can provide precise and effective evidences with high time resolution for glioma grading, assessment of peritumoral involvement and glioma therapies.</p>

ABSTRACT

Objective To analyze and evaluate the characteristics of cerebral sparganosis on MRI. Methods MRI appearances of 15 cases of cerebral sparganosis proven by pathology or serological test were retrospectively studied.Results A total of 21 lesions were found,some of them were single and some were multiple but asymmetrical.8 lesions were found in parietal lobe,6 in frontal lobe,2 in temporal lobe,2 in oceipital lobe,1 in basal ganglia,1 in cerebellar hemisphere and 1 in pons.The lesions carried slightly hypointense on T_1-weighted images and moderate hyperintense on T_2-weighted images.Most lesions showed pefilesional brain parenchyma edema.Enhanced MRI was performed in 10 cases,and all of them demonstrated abnormal enhancement such as peripheral ring-type,tortuous beaded shape or serpiginons tubular shape enhancement.Follow-up MR images in 6 eases were analyzed,and the location and shape of the lesions changed in 2 of them.Conclusion There are some specific MRI findings on cerebral sparganosis.Enhanced MRI and follow-up examination are important for the diagnosis of cerebral sparganosis.
